We are in-network with most PPO plans as well as Medicare.. As a courtesy to you, our team will contact your insurance provider for a quote on coverage details.

We offer flat-rate pricing for Physical Therapy sessions. $175 for initial consultations and $150 for 60 min follow up visits, $100 for 30 min follow up visits. We will provide documentation/ a super bill to submit to your insurance towards your out-of-network benefits. As a courtesy, our office will contact your insurance provider to attain specific out of network benefits.

Each patient’s condition is different. As a team (patient, PT and referring physician if applicable) will determine an effective plan of care that takes into consideration your diagnosis, your goals, your time as well as the financial implication.

We have unlimited patient parking in the attached parking garage immediately north of the building. Entrance to the garage is on the west side of Kingsbury, and the largest notable sign is for Extra Space Storage. Park anywhere on the first floor, including spots marked Extra Space.

After parking anywhere on the first floor, head to the small silver elevator bank in the southeast corner of the garage (closest to the Kingsbury entrance, opposite to the garbage/ recycling fencing). Do not pass through double sliding glass doors towards green elevator banks (those will bring you into the Extra Space storage facility). At this time, you will need to enter an elevator code (*5942#) then hit floor 3. Pass through the first double glass doors, and veer slightly right. Head straight down that hallway until you see Suite 307.
