Owner, Board Certified Clinical Specialist in Orthopaedic Physical Therapy
Molly completed her Bachelors in Biomedical Sciences in 2007, and her Doctorate in Physical Therapy in 2009 from Marquette University. She then went on to attain her Board Certification in Orthopedics in 2016 making her an Orthopedic Certified Specialist after years of dedicated continued education. She is also certified in Trigger Point Dry Needling, Graston Technique and Manual Therapy. During her fourteen years practicing in orthopedic and sports medicine settings, she has gone on to be an active member of the sports medicine team for Team USA with the USOPC as well as consultant for amateur (triathletes, marathoners, Iron Men and Women) and professional athletes (MLS, NBA, NHL and NFL). She has dedicated her professional career to providing patients a pathway to return to the activities that bring them joy.

Physical Therapist
Casey completed his Bachelors in Exercise Physiology from Marquette University in 2014 and his Doctorate of Physical Therapy from University of Saint Mary in 2019. Casey has worked with athletes at all levels, having served professional athletes as the Team Physical Therapist for Chicago Fire FC as well as collegiate athletes as the physical therapist for DePaul University Division I Athletics. Certified as a Functional Golf Specialist and in Triggerpoint Dry Needling, Casey tailors treatment plans to optimize an individual’s performance by enhancing mobility and strength of the entire body. Outside of sports medicine, Casey has had great success treating clients in post-operative care and complex spine conditions. When Casey is not in the clinic, he is enjoying his time with his wife and daughter, participating in basketball and softball leagues, or you can find him out on the golf course.

Clinic Administrator
Stacey Koch is the clinic administrator at The Forem. With more than 15 years experience in health care and customer service, Stacey is committed to The Forem's mission of true, supported recovery and patient-focused care. With previous experience as a CNA, she desired to stay in a field that allowed her to continue to care for others. When she's not greeting patients in The Forem's sunlit office, you can find her walking her dog Phil around her neighborhood in Logan Square while enjoying a tasty treat or two. You can also find her rooting for her favorite sport and team, the Chicago Blackhawks!
