Looking for a physical therapist (MPT, DPT)  to join our practice who thrives on individualized and focused patient care, collaboration with the leading sports medicine practitioners in the city, and a team mentality that isn’t bogged down with unrealistic budgetary/visits-per-day obligations based on a corporate entity mindset.

The Fôrem - is a private practice in the heart of the Lincoln Park neighborhood, immediately northwest of the city center of Chicago. Clientele is predominantly sports medicine/orthopedic, with opportunities for TMD, post-concussion/vertigo, and outpatient neuro, if desired.

The clinic prides itself on one-on-one, full-hour sessions, without the use of PT aides/techs in the administration of care. Complimentary indoor parking is attached to the clinic for both clinicians and patients.

South-facing, third-floor location with sweeping views of the skyline and Chicago River. Steps from the Red Line Clybourn stop, North Ave and Clybourn bus lines, as well as the Metra UP-NW Clybourn stop. Clinic hours are 7am-6pm M-F, no weekend obligation or mandatory 8pm expectation traditional to the outpatient model. Flexible hours.The Fôrem opened its doors in July 2022 in a brand-new custom-designed space, with equipment and modalities reflective of the most clinically relevant and up-to-date clinical research.
